
Financial Arrangements
Because you are a valued Valley Dental Group patient, we want to inform you
of the financial arrangements available at our office.
Our Philosophy
Our goal is to provide Valley Dental Group patients with a variety of financial
arrangements that allow them to receive and pay for quality care in a manner
that best suits their personal situation.
Schedule of Savings
Day
of Service Payment: 5% cash savings when paid in full the day
treatment is provided and there is no balance on the account.
Pre-Payment:
- 7% discount for pre payment of treatment 2 weeks prior
to
finished work
Patients Age 65+: 10% cash savings when pre-paying or for
payment in full the day of treatment and there is no balance
on the account.
(Note: savings not available for over the counter products;
savings not available to Delta Dental patients due to contract
limitations)
WE ACCEPT CASH, PERSONAL CHECKS, VISA, MASTERCARD, AMEX
& DISCOVER.
Financial Options
Valley Dental Group offers several third party financing options. These options
range from revolving credit lines to fixed payment loans with interest for up to
84 months for qualified patients.
Installment Plan
Finance charges can be avoided when an initial payment of
33% or 50% is made at the start of treatment and the balance
is paid in two equal payments following treatment. Please
speak with our financial coordinator either over the telephone
or in the office prior to the start of treatment to set up
the payment schedule.
Finance Charges
A finance charge of 18% annually (1.5% per month) will begin accruing on any
unpaid balances after 60 days.
Payments/Delinquencies
All monthly statements are due and payable upon receipt unless prior financial
arrangements have been made. We accept cash, checks, and most major credit
cards. Patients with delinquent accounts are liable for all costs incurred for
collection of past due balances, including collection agency fees, attorney fees,
conciliation court fees and all costs involved in litigation.
If You Have Dental
Benefits
Financial arrangements are necessary for balances exceeding
$250 after the benefactor had paid its portion. Our financial
coordinator is available to discuss your estimated dental
benefit, annual maximum, deductible, and financial arrangements
for the amount not covered.
Prosthetic
Case Deposits
For dental treatment requiring prosthetic cases (crowns, bridges,
veneers, etc.), a $200 per tooth deposit is required. Unless
other arrangements have been made, complete and partial dentures
require 50% down payment prior to the start of treatment with
the balance due upon completion.
